After some time, technical systems have been changed again and again. Thereby, they became more complex; the longer the history, the more complex they usually are. But with increasing complexity, safety and security risks - and maintenance costs - also increase. So, from time to time, some 'gardening' is recommended.
However, with complex systems, we need to be careful: thorough brain work (analysis) together with all stakeholders is needed before any action is started (and the actions should be started in small steps).

About me / Lessons learned
In a nutshell, I'm a seasoned German Sicherheits-Guru (safety and security) who worked with many companies, with some in a permanent position - e.g. as Head of Quality of a mid-sized company, reporting to the CEO - but with most as a freelancer for product and/or system sicherheit. Most of the lessons learned (almost always the hard way) are that sicherheits problems (and nonsense costs) are mostly caused by the complexity of a system that has grown in a non-controlled way.
If the break controller of a vehicle suddenly also takes care of the HVAC, or if mostly old (non-patched) production equipment and/or the most valuable data of a company are suddenly accessible via the internet, the complexity and therefore also the risks and the maintenance costs become issues.
